HWMonitor version 1.31 now supports Ryzen

HWMonitor version 1.31 now fully supports AMD's Ryzen architecture, delivering readings for CPU package temp, per-core and package power and proper VID readings.

This update also updates the program's support for Polaris GPUs, providing more in-depth power information. Previously the program only provided limited data on modern AMD hardware, providing no CPU temperatures outside of the data that the motherboard's sensors provided.
