Moving from Low to Medium presets in Overwatch presents the largest visual change, increasing the environmental detail greatly by adding new objects all over the level from chairs and keyboards around or on desks, vents on the walls and even hanging lights in the ceiling.   

Moving from Low to Medium also sees shadow resolution become much more defined and see a lot of objects start to reflect light realistically, well, at least as realistically as a cartoon-style world can be. 

Beyond medium settings we see lighting and shadow resolution continue to improve, with the environment losing the blurring effect of FXAA as it is replaced with SMAA.

Overwatch is clearly designed to be played at medium settings or above, with the game at lower settings lacking a lot of the small details that make the world feel like an active “lived in” place rather than a dull and lifeless level design.
